THE ROLE:
As a Case Manager, you will support the in-home clients receive a high standard of care, managing (clinical + non-clinical) staff, and support staff to achieve this. Your day-to-day role will include:

• Creating, adjusting and presenting care plans for clients
• Working with clients, family members, and service providers to create the best support for the individual
• Support staff with clinical questions, and help with overseeing documentation
• Collaborating with staff to ensure they are supported, heard, and are enjoying their role
• Ensure you are up to date with the quality and safety standards within home care

MUST HAVE:

• At least 2 years of experience within Home Care in Australia
• Proven understanding of and commitment to aged care accreditation requirements
• Current National Police Check and NDIS Screening with no disclosable outcomes
• Up to date compliance for the Aged Care sector including mandatory vaccinations
• Valid working rights in Australia
